Sister Other Paranoia follows a university student majoring in literature who has a strange, synesthesia-like ability that allows him to see other people’s emotions as colors. After a light novel he writes unexpectedly wins an award, his unusual gift becomes even stronger. He soon finds himself able to read the minds of those around him, revealing thoughts filled with envy, jealousy, and malice.

As the experience takes a toll on him, the protagonist gradually isolates himself from the outside world. His younger sister becomes the only person he continues to trust, creating a relationship that sits at the heart of the game’s unsettling story.

Rather than telling a simple, straightforward tale, Sister Other Paranoia puts players inside the protagonist’s increasingly distorted perspective. Their choices influence relationships and can lead to several disturbing outcomes, while the game avoids offering clear answers about who is right or wrong.
